    Are you under 18 years old?
  If NO, please continue.
  If YES, your parent or guardian must fill out and sign our Medical Authorization form.
   
    Is a sponsor in the United States paying for your study?
  If NO, your application is finished!
  If YES, you must fill out the U.S. government Affidavit of Support, form I-134.
